Learners with SEN/disabilities
For those students with a Statement of Special Educational Needs the annual review meetings following their 14th birthday will have a focus on transition. The school will organise a Transition Review and students and their parents will be invited to attend the meeting along with the Connexions PA and any other services actively involved with the student at that time.
Reviews in Year 10 and Year 11 will continue to focus on transition so that appropriate options are identified. The Connexions PA will complete the Section 139a Assessment for all students with Statement of SEN and for those without a statement but have a Learning Need that will require additional support when moving to education, employment or training. This key document is a summary of the students needs and provision, giving potential providers a complete picture of the student enabling them to prepare appropriate packages of support where needed. The Connexions Personal Adviser will use this document to support the young person and ensure that any additional support needs are addressed.
